I also don’t understand what you mean. If you are a US citizen, then (at least for tourist travel anyway) you don’t need a visa to visit Italy or Portugal for a max of 90 days out of 180 days.
You will however need a passport for identity purposes. If you fly, you will need to provide your passport. On the train/bus, you may (but not always) be requested for it as well
All foreigners in Italy are required by law to have an ID with them at all times so you would need to show it anyway upon request.
If you are a foreign legal resident in Portugal and have a visa for this, then the terms of this visa dictate whether you can travel onwards or not, and it may be wise therefore to check with the Portugese immigration dept or Italian embassy.
